- CDC National ART Surveillance System clinic datasets, reporting years 2020–2022; NPI Registry; clinic websites.
- Volume estimate = the CDC's reported all-ages share of transfers that used a gestational carrier × the program's total transfers (the transfer count the CDC uses as that share's denominator). Both figures come from the clinic's own CDC dataset for that year.
- CDC: "Comparison of success rates across clinics may not be meaningful due to differences in patient populations and ART treatment methods."
- Where the CDC suppressed the all-ages carrier share, the estimate instead weights the reported age-group shares by the program's own-egg transfers in each group and scales the result to total transfers; such estimates are marked on the clinic page.
- Clinics the CDC lists as non-reporters for a year have no figures for that year; their pages say so and they are not ranked.
